Healthy Family Support Practitioner - Part-time Vacancy working with children and/or Young People aged 0-9 years and their families in the Rochford Area.
We are seeking a passionate, dynamic, and experienced practitioner to join our Healthy Family Team. Our teams are fully integrated, consisting of Health Visitors, School Nurses, Support Practitioners, Family Hub Practitioners, and Community Connectors.
Role Overview
To work with children, young people, and their families on a one-to-one or group basis, using various interventions to identify needs and achieve positive outcomes. To collaborate with parents/carers to empower them and improve outcomes for their children. To work cooperatively with children's services, health agencies, and community services to provide effective support.
Key Responsibilities
1. Undertake developmental and health reviews at key stages, including occasional reviews for children aged 0-5 years.
2. Support delivery of the Healthy Child Programme and Maternal Early Childhood Sustained Home Visiting (MECSH).
3. Promote attachment, the Baby Friendly Initiative, and support families through group sessions.
4. Deliver evidence-based parenting support and interventions to support children’s development and school readiness.
5. Organize and participate in two-year reviews with families and early years settings.
6. Participate in virtual sessions to ensure all families are reached and supported.
7. Provide brief interventions and advice where parental conflict is identified as a risk factor.
8. Lead health promotion activities in family hubs and external venues.
9. Support emotional literacy and wellbeing through coaching and skills training.
10. Deliver educational sessions tailored to the audience.
11. Partner with specialist providers to support families of children aged 6-19, including those affected by domestic abuse.
Essential Requirements
* Minimum NVQ Level 3 qualification.
* Experience working with children in school, home, and community settings.
* Confidence in delivering advice/support to parents and carers.
* Good record-keeping and understanding of GDPR.
* Empathy and experience building relationships with vulnerable children and adults.
* Ability to drive and access to a vehicle.
