Deliver Meaningful Life Programs to our residents! Leadership role offering Full time hours, working Monday - Friday. Located in Sandringham.

About the role:
* Monday - Friday, 9am - 5pm
* Lead the Meaningful Life program in the Care Community
* Develop opportunities for meaningful engagement, purposeful participation, social events and activities for all residents
* Drive Opal HealthCare initiatives including Meaningful Mates and Helping Hands
* Curate programs to support residents’ physical, emotional and spiritual wellbeing and overall quality of life
* Develop resident care plans and complete key lifestyle assessments
* Work together with clinical teams and allied health professions provide a tailored approach for each resident to aid their mobility and independence
* Coach care team members to deliver meaningful engagements between like-minded residents
* Liaise with residents to develop activities and engagements that meet their cultural, spiritual, social and physical needs and preferences
* Engage with residents’ families to encourage visitation and socialisation
* Demonstrate Person Centred Care principles approach
