Adult’s Social Worker
Team: Neighbourhood
Location: Bolton
Details: Bolton are looking for a SW for the Neighbourhood team any age adult SW 18+ with any need other than LD. The candidate will be working with health professionals such as district nurses, therapists, etc. Candidates must have 2 years experience within a specific neighbourhood team – otherwise 3 years experience+ is required. The role is 50/50 office and hybrid working but candidates will be fully office based for the first 4 weeks and must be local to Bolton and able to commit to working in Bolton as the role is hybrid but must be able to make it in to the office or complete a visit at short notice.
An exciting opportunity has arisen to join Bolton’s Adult Central South Neighbourhood team. Within the role, you will be expected to work closely with our health colleagues to provide support to adults and their carers who may be eligible for assessment and care services under the Care Act 2014.
Role context

Responsibilities
* Undertaking assessments and providing ongoing support to meet the needs of service users under the Care Act.
* Devising care and support plans, completing reviews, and conducting Mental Capacity Assessments.
* Participating in Best Interests meetings and working with people with complex social needs.
* Ensuring people in Bolton receive the support that meets their needs and proactively addressing complex issues, family dynamics, legal issues, and crises.
* Providing information and advice; transferring cases to other teams as required; attending case conferences and reviews; representing the Council at Court when appropriate.
* Safeguarding function including assessing and investigating risk and undertaking Safeguarding Adult Enquiries as required.
* Utilising resources from Social Care, Health and other voluntary and statutory agencies to provide short-term and long-term assessments of independence and ongoing support needs.
Working arrangements
The role requires completion of related administrative functions and being available to attend case conferences and reviews. Local to Bolton with the ability to commit to in-person visits and short-notice requirements. 50/50 office and hybrid working, with full office base for the first 4 weeks.
Essential qualifications and requirements
* DIPSW or equivalent qualification in Social Work
* Social Work England registration
* Enhanced and in-date DBS certificate
* Excellent communication skills and the ability to make good decisions based on information available
* Ability to work alone and within a team/multi-agency settings
* Knowledge of service needs
* Competent IT skills and ability to navigate electronic file/recording systems
* Ability to write clear, concise, and robust assessment/reports/files
* Flexible approach to work and working hours when required
