Join to apply for the Band 5 Community Nurse/Paramedic role at Sussex Community NHS Foundation Trust.
Our Urgent Community Response (UCR) Service provides urgent support, seven days a week, to help prevent unnecessary hospital admissions. This is achieved by delivering a two-hour crisis response in the community by a multi-skilled team of professionals. Support is provided at home or where people reside when concerns about health or mobility indicate a risk of admission to hospital. Our teams work closely with the local ambulance service to pull category 3 & 4 patients off the ambulance stack, enabling a rapid community response by teams with skills to help patients remain at home.
The UCR service also provides a general virtual ward service, supported by digital technology and remote monitoring. Multidisciplinary teams ensure that patients receive hospital-level care at home safely and in familiar surroundings, helping speed up recovery while freeing up hospital beds for those who need them most. In addition to home visits, digital remote monitoring is utilised.
Teams are made up of GPs, Nurses, Paramedics, Pharmacists, Occupational Therapists, Physiotherapists, and Health & Therapy Assistants who work together to reduce hospital admissions. Patients are reviewed daily by a clinical team, and ward rounds may involve a home visit or take place via video technology.
As a Nurse or Paramedic, you will deliver holistic assessments, plan, implement, and evaluate treatment care plans, alongside reviewing care provision. You will provide input for patients, including identifying and managing deterioration in the community by using a range of core skills. You will be part of a team supporting evidence-based care, facilitating staff learning and development.
This post is an ideal opportunity for a nurse or paramedic who is keen to develop holistic assessment skills and knowledge of urgent community response. If you are new to Sussex Community NHS Foundation Trust, you will join our preceptorship programme with protected time and regular clinical supervision with dedicated registered professional support.
