Overview
Radford Medical Practice is seeking a Care Coordinator who is passionate about supporting others and making a real difference every day. This role is ideal for someone who is proactive, highly organised and motivated. As a Care Coordinator, you will play a key role in supporting patients in receiving personalised, coordinated care. You will work closely with Primary Care Network (PCN) colleagues to identify and manage a caseload of patients. We are keen to attract a caring and passionate individual to join our experienced team. The post holder will be required to work across our two surgeries covering opening hours of 8am - 6.30pm for 37.5 hours per week.
Main duties
As a patient-contact role, the post holder will be responsible for proactively identifying and supporting patients and their carers in the development of personalised plans, making appointments and in the coordination and navigation across health and care services.

We are a long standing, busy inner city surgery based across two sites. We currently have around 20,000 patients with a large student population based at the Nottingham Trent University and a smaller diverse population based at our inner city Radford site. * To proactively identify and support patients who may benefit from care coordination (e.g., frail patients, people with multiple long-term conditions, frequent service users, or those requiring end-of-life care planning)
* To help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care
* To help patients and their families navigate health and social care systems
* To maintain accurate records on clinical systems ensuring data quality and confidentiality at all times
* To have excellent interpersonal and communication skills, with the ability to work sensitively with diverse groups
* To have strong organisational skills with the ability to manage multiple priorities and work independently
* To be competent in the use of IT systems including Microsoft Office
