The objective is to assist them in making the transition from a chaotic street lifestyle to sustaining stable accommodation[/li][li>Co‑provision of support/resettlement plans, risk management plans, crisis intervention, and engagement with customers in order to deliver positive outcomes in a service that offers involvement and empowerment for customers[/li][li>To provide a high‑quality service, ensuring outstanding customer service and service delivery by acting as an ambassador for Humbercare and a point of contact for customers, working in conjunction with the Homelessness Prevention Service and other voluntary and statutory services such as Rough Sleeper Initiative‑funded partners, mental health services, social care, and accommodation and support providers.
