Overview
An opportunity has arisen for a dynamic, enthusiastic, self-motivated, experienced Clinician, with good clinical palliative care knowledge base, to join our well-established Hospice Community Team as a Development Clinical Specialist. This post will provide varied senior experience in a multidisciplinary working environment delivering a 24/7 service to individuals, carers and other health care professionals across the community.
With the launch of the hospice 5-year strategy and ranked Outstanding by Care Quality Commissioners, there is a strong platform to join an experienced & skilled specialist palliative care service, providing excellent, compassionate care to individuals and their loved ones across all residential community settings.
We are looking for a candidate who understands the needs of those receiving palliative care/end of life care, a working knowledge of evidence-based practice and the ability to develop a good knowledge base of current initiatives and developments in palliative care. The candidate will possess excellent communication skills, the ability to progress autonomy within a team environment and develop caseload management skills.

We achieved Outstanding in our CQC inspection & have reconfigured our services to meet the changing needs of our local community. We support 5 Clinical Commissioning Groups & have a robust governance structure in place. We provide ward clinical services within a modern, personal & friendly 18-bedded unit, as well as Specialist Community Crisis Support Service, Hospice at Home Service & Specialist Day Therapy Service. We also have the hospice Pepperell Education Centre that has an excellent reputation for delivering high quality education programmes and study days.
Position Details
Hours: 37.5 hours per week; Service runs 7 days a week over 24 hours 365 days a year; Reports to Community Team Lead; Key Relationships listed below.

Responsibilities
* To provide appropriate end of life care addressing physical, emotional & spiritual care in agreement with the individual & their loved ones following development of skills to undertake complex, holistic assessment of need
* Take responsibility for own safety in the community when working autonomously & adhere to lone working policy & procedures as set by the organisation
* To be responsible for travel to & from individuals home & plan & organise community visits efficiently
* To communicate effectively and appropriately using advanced communication skills, with individuals, their carers & liaise with other professionals involved in their care on behalf of the Hospice Community team
* Develop skills in recognition of palliative care emergencies through clinical symptoms and assessment. To take responsibility to recognise & report changes in an individual’s condition & respond appropriately to maintain effective symptom control
* Accurate & timely recording of assessments & clinical history
* Understand the importance of multi-professional team working & the ability to promote it within the organisation and externally
* To participate in the service delivery over a 24/7 365-day service as directed by the Development Mentor & Community Team Leader with weekend & on-call work being part of the rota
* Take responsibility to develop & establish external relationships with District Nursing teams, GP practices, acute sector & other hospices/specialists
* Attend monthly Hospice Community Team meetings & weekly multidisciplinary team meetings; attendance at Reflective practice sessions will be required during development
* Demonstrate progression from intermediate to advanced clinical knowledge of the principles & practice of palliative care through competency achievements & ongoing self-reflection
* Maintain HCPC or other professional registration & comply with relevant professional standards
* Attend and represent Saint Francis Hospice at internal/external meetings as directed
Qualifications
Essential
* Professional Health Care qualification with significant post-registration experience (RGN or HCPC registration as an Allied Health Professional, or Paramedic)
* Oncology/Palliative Care Qualification / Relevant Skills/Experience in Palliative Care
* Minimum 5 years post-qualification experience
* Non-Medical Prescriber or willing to undertake
Desirable
* Teaching qualification/experience Level 3 and above
* Degree or equivalent experience or willingness to undertake
* Non-Medical Prescriber or willing to undertake
Other Requirements
* Efficiency to travel around designated areas and flexibility to work in a 24/7 service
Professional/Experience Details
* Knowledge of current issues in specialist palliative care and equity of access
* Flexible approach to team working and an awareness of team dynamics/behaviours
* Ability to assess situations comprehensively and decisively plan care
* Motivated and enthusiastic willingness to develop advanced clinical skills
* Ability to flourish in a rapidly changing/developing service
* IT knowledge and use of electronic records with awareness of confidentiality and GDPR
* Significant experience at Band 6/equivalent senior level in palliative care or related field
Professional Responsibilities
* Act within Codes of Professional Conduct (HCPC or NMC)
* Maintain own professional development and participate in supervision and annual appraisal
* Non-medical prescribing after course approval, if applicable
* Engage in MDT meetings in primary/community care settings with GPs and Care Homes
* Adhere to health and safety policies, lone working policy and fire regulations
