Social Prescribers support other health and careprofessionals with an option to refer patients to a range of local, non-clinical services when their health can no longer be improved through traditional medical interventions.

Our Social Prescribers supportindividuals to take greater control of their own health in a way that makessense to them and enables them to feel connected and supported to facilitatesustainable change.

Social prescribing schemes can involve a variety ofactivities which are typically provided by voluntary and community sectororganisations in locations that feel comfortable for the individual: activities that supportfeelings of contribution and connection with others.

We recognise that community-centred ways of working can be more effective than more traditional services inimproving the health and wellbeing of individuals and have a good track-record of providing this.

Please be aware that our organisation does not hold a visa sponsor licence, so we are unable to accept applications from those requiring visa sponsorship to work in the UK.

Main duties of the job

We arelooking for a caring, empathetic and sensitive individual to help people of allages, to improve their health and wellbeing. To do this you will have excellent listening skills and use your knowledge and experience to help support people to engage and identify their own support methods. This will involve working in a multi-disciplinary waywith GP Practices to build networks with voluntary sector organisations, localauthorities and other partners. You will need to be proactive to identify and approach these organisations to develop options for our patients.

You will be a confident individual who is comfortable speaking and meeting people from a range of backgrounds. You will be skilled at managing a caseload of individuals and seeking support and advice, when necessary, but be able to work autonomousy.


About us

The North West Leicestershire GP Federationconsists of 12 GP Practices covering 116,000 patients. The GP Federation operates theNorth West Leicestershire Primary Care Network.

At North West Leicestershire our team of Social Prescribers work in our three neighbourhoods, to support our practices and patients. We want to support the development of the team and provide additional capacity as the service grows. You will primarily support in one of our neighbourhoods, which is a smaller geographical area, but would also help cover in any of our 12 practices when needed.

Job responsibilities

Receivingand actioning referrals from a wide range of agencies, working closely with 12GP practices within North West Leicestershire primary care network (PCNs),pharmacies, multi-disciplinary teams, hospital discharge teams, allied health professionals,fire service, police, job centres, social care services, housing associations,and voluntary, community and social enterprise (VCSE) organisations. (List notexhaustive).

Providingpersonalised support to individuals, their families and carers to enable themto take control of their wellbeing, live independently and improve their healthoutcomes. Develop trusting relationships by giving people time and focus onwhat matters to them. Taking a holistic approach, based on the person'spriorities and the wider determinants of health. To co-produce a personalisedsupport plan to improve health and wellbeing and to introduce or reconnect peopleto community groups and statutory services. It is vital that the SocialPrescribing Link Worker has a strong awareness and understanding of when it isappropriate or necessary to refer people back to other health professionals oragencies.

Toincrease the strengths and capacities of local communities, and enable localVCSE organisations and community groups to receive social prescribingreferrals. To ensure patients are supported, have basic safeguardingprocesses for vulnerable individuals and provide opportunities for theperson to developfriendships, a sense of belonging, and build knowledge, skills and confidence.

Worktogether with all local partners to collectively ensure that local VCSEorganisations and community groups are sustainable and that community assetsare nurtured, by making them aware of small grants or micro-commissioning ifavailable, including providing support to set up new community groups andservices, where gaps are identified in local provision.


Person Specification


Qualifications

* GCSE Grade C or above in Maths and English, or equivalent qualification.
* Qualification in a health or social care.


Experience

* Experience of working in a similar role.
* Experience of working with vulnerable people.
* Experience of working in health and social care.
* Experience of coordinating services from diverse providers.
* Knowledge of the needs of vulnerable adults, safeguarding and the associated legal framework.
* Knowledge of local health and social care provision.
* Knowledge of funding systems in social care.
* Knowledge and understanding of GP Practices.
* Ability to manage and prioritise a caseload
* Ability to work flexibly and enthusiastically within a team or on own initiative.
* Good communication skills, both written and verbal.
* Ability to build relationships with patients, their families and carers.
* Ability to provide personalised support to individuals, their families and carers.
* Ability to listen and empathise with people in a non-judgmental way.
* Able to provide leadership and complete tasks in a timely manner.
* Able to maintain effective working relationships and promote collaborative working.
* Experience of working in liaison capacity with social care.
* Experience of seeing patients and carers in a practice based setting
* or in their own home.
* Knowledge of public health issues.
* Familiarity with information systems used in clinical practice.
* Understanding of the role and function of GP Federations and Primary Care Networks.
* Understanding of health and social care terminology.
* Ability to communicate difficult messages to patients and families.
* Experience of using clinical systems such as SystmOne / EMIS Web.


Other

* Full UK driving licence.
* Meet DBS reference standards.
* Highly motivated.
* Willingness to work flexible hours when required to meet work demands.
* Able to demonstrate good time management skills.
* Undertake additional training relevant to the role.
