Position Overview
An opportunity has become available to work within the Care4CE Mental Health Reablement Team. The team offers support to adults with mental health issues using the reablement model. This is a permanent position for Grade 6, 16 hours a week. Hours are primarily 9am – 5pm, although some early evenings may be required. The office base is Macclesfield Town Hall, but the role requires visiting referrals at their homes and transporting to community resources – a mileage rate is paid for the use of a vehicle. Referrals to the team come from various sources (CMHT, social workers, GP, mental health teams at the hospital). The team also supports users with social care issues such as housing, budgeting, and social isolation, encouraging mental health wellbeing through coping strategies and self‑help booklets.
Key Responsibilities
* Provide support to adults with mental health challenges using a reablement approach.
* Collaborate with referral teams (CMHT, social workers, GP, hospital mental health teams) to develop and implement care plans.
* Visit clients in their homes and arrange transport to community resources.
* Assist users with social care matters: housing, budgeting, reducing social isolation.
* Encourage wellbeing by sharing coping strategies and self‑help booklets.
* Maintain accurate records and reports in the system.
Qualifications and Experience
* Experience working in a social care or health care setting, ideally with mental health referrals.
* Experience supporting service users and carers and building good working relationships.
* Strong communication and teamwork skills.
