Our dedicated teams work together to ensure the very best standards of care, and we are now looking for a passionate and motivated Team Leader and Care Plan Lead.
The Role
This is a key role within our home, combining hands-on leadership with responsibility for care planning and compliance. You will:
* Lead and supervise the care team on shift, ensuring high standards of person-centred care are delivered.
* Take the lead in care planning and reviews, ensuring all care plans are accurate, personalised, and regularly updated.
* Administer, record and monito medications in line with policies and procedures.
* Support staff with training, development, and mentoring to maintain best practice.
* Act as a point of contact for residents, families, and external professionals.
* Monitor compliance, safeguarding, and quality standards in line with CQC requirements.
* Contribute to audits and continuous improvement initiatives within the home.
About You
We are looking for someone who is both a natural leader and highly organised, with a strong focus on care planning. You should have:
* Previous experience as a Senior Carer, Team Leader, or similar role in a residential care setting.
* Strong knowledge of care planning, risk assessments, and person-centred care.
* Excellent communication and leadership skills, with the ability to motivate and support a team.
* Good IT and record-keeping skills.
* NVQ Level 3 in Health & Social Care (Level 4 or working towards is desirable).
