Are you interested in being part of the Community Care Team Integrated Team based at Cowal Community Hospital, Dunoon? If so this is the post for you. Our overriding aim is to support individuals to maintain their independence at home and support people to regain independence after they have left hospital.
We are looking for motivated & enthusiastic individuals who will be part of a multidisciplinary team working with patients/clients within their own homes/homely setting to provide holistic care. Your primary responsibility will be to assist with enabling, prevention of deterioration and post hospital holistic care within a reablement approach.
The post holder needs to be flexible and able to use their own initiative as well as provide high standards of patient care in support of the team to ensure delivery of a high quality service to the local community.
