Position Summary
The Learning Disabilities Care Hub, part of the Specialist Network within Pennine Care NHS Mental Health and Learning Disabilities Trust, is recruiting Senior Support Workers for Cambeck Close short breaks. This exciting opportunity involves supporting children and adults with learning disabilities, with or without autism, who are accessing short‑break provision.
We are looking for individuals with excellent interpersonal skills. The post holder will provide care and support to service users, their families and carers, and will also offer supervisory support to other Support Workers, working closely with the Unit Manager, Administrator and other professionals across the Bury area.
The Senior Support Worker will be a role model, demonstrating high‑quality, person‑centred care and collaborating with the Service Manager to manage the service effectively. The role includes ongoing assessment, care provision and monitoring in line with agreed pathways and supervision, maintaining high standards and ensuring all relevant plans are implemented during shifts.
Key Responsibilities
* Under the supervision of a Unit Manager, provide and deliver a high standard model of care, contributing to ongoing assessment and monitoring based on evidence and working with the multi‑disciplinary team.
* Provide compassionate, safe and effective care and support to clients.
* Monitor the physical, mental, emotional and behavioural health and wellbeing needs of service users continuously, ensuring appropriate escalation when required.
* Support service users to improve and maintain their mental, physical, emotional and behavioural health and well‑being.
* Promote comfort and wellbeing by ensuring that service users' personal and social needs are met, and provide nursing care as directed within individualised care plans alongside the multi‑disciplinary team.
* Prioritise and manage own workload, recognising where tasks may be safely delegated to colleagues, family members or carers.
* Provide and receive sensitive information concerning a service user’s medical and/or emotional condition.
* Work with the multi‑disciplinary team and engage with other agencies and community teams to ensure a safe, individualised approach to admission and discharge.
* Maintain accurate, contemporaneous service user records in line with Trust policy.
* Ensure interventions are provided within a least restrictive environment where possible, delivering evidence‑based care under indirect supervision of the Service Manager.
* Communicate effectively with dignity and respect to all service users, their families and carers, considering potential barriers to understanding.
* Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions during delegation or hand‑over of responsibilities.
* Recognise and report any situations, behaviours or errors that could result in poor care outcomes; report accidents, incidents, near misses or complaints following Trust policies and procedures.
* Contribute to a culture where all staff feel safe to raise concerns and speak up as a normal part of routine.
* Recognise the effects of medicines, allergies, drug sensitivity, side effects, contraindications and adverse reactions; undertake safe and effective administration and optimisation of medicines in accordance with Trust policy.
o Administer medicines via oral, topical, rectal, subcutaneous and intramuscular routes in line with Standards and Trust policies.
o Understand escalation processes and seek timely advice for concerns, including emergencies.
o Attend mandatory training related to managing physical health needs and medicines management, maintaining compliance with required training.
o Monitor the condition and health needs of service users following medication administration.
o Recognise and report any medication‑management related situations, behaviours or errors that could result in poor care outcomes.
* Comply with mandatory competencies in line with Trust policy.
* Maintain own continuing professional development and contribute to formulation of personal objectives and development plans.
* Support, supervise, teach (where appropriate) and act as a role model for support workers and new staff, promoting reflection and providing constructive feedback.
o Contribute to the maintenance of a learning environment, adopting a reflective approach to own practice with a view to continual improvement.
o Undertake further training as required, prior to carrying out any additional duties identified as relevant to the role, to comply with Trust policies and procedures.
