Street Triage Practitioner - Cheshire West
Pay: £38,682 - £46,580 per annum. Permanent, full‑time. Location: 71-77 St Annes Street, Chester CH1 3HT.
Job Summary
Support the Street Triage Team in Cheshire West by providing comprehensive assessment and intervention for people experiencing mental health crises in the community.
Key Responsibilities
* Conduct comprehensive assessment for individuals in crisis, working in partnership with police and other services.
* Specialise in the inter‑relationship between physical illness, psychosocial disturbance and mental health problems.
* Provide mental health and risk assessment for people presenting with self‑injury or self‑poisoning.
* Advise on and contribute to treatment and management plans in collaboration with multi‑disciplinary colleagues.
* Deliver training to staff and police on mental illness and appropriate responses.
* Participate in Urgent Care Mental Health First Response service including Crisis Triage, Liaison Psychiatry, Home Treatment Teams and Street Triage.
Qualifications & Experience
* Registered Mental Health Nurse (RMN) or equivalent professional registration.
* Extensive experience working in varied mental health settings and multi‑disciplinary teams.
* Knowledge of sensitivity to equal opportunities and current professional development needs.
* Experience in managing physical pathology and the link between physical and mental health.
* Desirable: Evidence of further professional development; recognised qualification in psychotherapeutic interventions, e.g. postgraduate CBT certificate.
* Information technology skills and familiarity with agencies supporting community services.
