Key Responsibilities
* Conduct assessments of individuals and families to identify care needs and develop effective care plans.
* Collaborate with external agencies and service providers to deliver holistic support.
* Maintain accurate and timely case records in line with policies and procedures.
* Provide expert advice to service users and their families regarding available local services and resources.
* Work in partnership with fellow social workers and multidisciplinary professionals to implement sustainable community care strategies.
* Participate in regular training to maintain up-to-date knowledge of current legislation and best practice.
Required Skills & Experience
* 1–3 years0experience within a fast-paced neighbourhood team.
* Strong understanding and experience of working with LAS (Liquidlogic Adults System) is desirable.
* A full driving licence and access to a vehicle is advantageous.
* All employment gaps must be accounted for and verifiable.
* Degree in Social Work and current registration with Social Work England.
