We are appointing Foot Health Practitioners to work in within the Podiatry and Diabetes service. We have vacancies across the 3 counties, Carmarthenshire, Ceredigion & Pembrokeshire.
As a foot health practitioner, you will be working in conjunction with Podiatry, Nursing, Medical and other members of the Multidisciplinary team within the Diabetic Foot Service. The role will be mainly to carry out diabetic foot checks, carrying out observations including bloods, and promoting healthy lifestyles.
These are permanent, part time roles, working in the Diabetes Foot Care departments.
The post holder will be expected to complete a focused in house training
programme along with the Agored Cymru (level 4) qualification which will provide the theoretical knowledge and practical skills to underpin the scope of practice required for this role.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The post holder will be responsible for the delivery of a high standard of diabetic foot assessments for patients across the integrated foot care pathway and administrative duties, as part of the integrated Diabetic Service in Hywel Dda University Health Board. They will also work closely with the Diabetic Specialist Nurses.
The post holder will undertake a job plan of work agreed between the Diabetes & Vascular Podiatry Service Lead and Lead Diabetes Nurse, providing a support service to inpatients, outpatients and will help develop wider diabetic foot services for the Diabetes department. They will be competent to work without direct supervision within an agreed framework whilst also being part of a supportive Multidisciplinary
team and will receive regular supervision and support.
The role will:
* Ensure that patients/clients receive safe, individualised and effective assessment of the diabetic foot by following care plans and sign posting to the relevant members of the Multi-Disciplinary Team;
* Ensure high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals;
* Ensure patient safety is maintained and patient experience is positive by treating all patients, family, relatives and colleagues with respect, dignity and courtesy in accordance with the Health Board values;

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.

* Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest
* Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire
* Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
* 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
* Numerous mental health and learning disabilities services
