The post holder will be responsible for coordinating the process of determining eligibility for Continuing Care within the parameters of the National Framework for Children and Young People's Continuing Care 2016 for children and young people registered within Bury/Stockport ICB.
To undertake functions, in accordance with the National Framework for Children and Young People's Continuing Care (2016), with particular responsibilities for individuals who have complex health care needs. This includes all categories of needs, e.g., physical, end of life, mental health, and learning disability needs.
All functions to be carried out jointly with individuals, families, carers, and other professionals from health and social care to ensure the health needs of individuals are identified and appropriately met.
Undertake nursing assessments and checklists.
Collate comprehensive evidence for Multi-disciplinary Team Meetings, organise and chair these meetings, complete DST, and prepare for presentation at panels. Roles include Children's Nurse/Neonatal Nurse.
Undertake comprehensive assessments, reviews, and complex case management in a specialist area (Continuing Care). Evaluate and analyse information gathered during assessments, which can often be complex and sensitive.
Provide a service to children and young people, including those with complex health needs, mental health difficulties, and/or learning disabilities.
Offer specialist knowledge across the range of work procedures and practices, underpinned by theoretical knowledge or relevant practical experience.
Undertake specialist Continuing Care and Personal Health Budget training.
Engage in self-directed learning regarding associated legislation, policies, and guidance.
Complete mandatory and job-specific training, including safeguarding children and adults, Mental Capacity Act training, and Deprivation of Liberty Safeguards training.
Gain ongoing experience in implementing the Continuing Care framework and managing Personal Health Budgets.
Provide training sessions for health and social care professionals. Support individuals and carers in understanding the Continuing Care assessment process, including those with cognitive impairments who may require adapted communication methods.
