What the Job Involves This role, positioned within the Adult Services Review Team at County Hall, involves arranging and monitoring services aligned with individual care plans. Responsibilities include providing advice, practical help, liaising with voluntary and statutory agencies, and completing annual care plan reviews. You'll actively participate in team meetings, duty rotas, and champion Council policies. While mostly office-based, some visits to service users may be required.
