The role

has become available to join a committed and motivated team who support individuals when they are ready to be discharged from hospital. The team work hard to deliver excellent standards in a positive working environment. The Hospital Team works in partnership with individuals to identify what matters to them. This includes undertaking Mental Capacity Assessments, Continuing Health Care Assessments, leading Best Interest Meetings to ensure people have a safe and timely discharge from hospital.
We work alongside a range of professionals within the teams, including colleagues from the health board, therapies and external providers. It is an exciting time to join adult social care with friendly, experienced and supportive colleagues who are dedicated to providing a service that puts citizens at centre of our work.

This post is subject to an enhanced with both adult & child barred list DBS disclosure
