Care & support Co-ordinator
2 posts – 6 month fixed-term contract
Responsibilities
* Be a key member of the Care Arrangement Team within the Commissioning, Contracts and Quality Service.
* Play a vital role in the procurement, brokering, and coordination of care and support packages across the community care market.
* Ensure residents receive timely, person-centred services that meet their assessed needs, while supporting the sustainability of the local care sector.
* Oversee the daily workflow, allocation, and prioritisation of referrals into Wirral's Care Arrangement Team to ensure smooth and efficient service delivery.
