Occupational Therapist

An exciting opportunity has arisen to apply for a 30‑hour/week, one year fixed‑term role as a Band 6 Occupational Therapist to work at the Ealing Referral Hub triage adult community rehab referrals. The role also involves some outpatient clinics for community rehab, e.g. post‑COVID rehab. Staff will be supported with clinical supervision, multidisciplinary team work, training and opportunities to take part in service improvement work.
We are looking for a candidate who has completed core clinical rotations and can adjust to a fast‑paced working environment. In this role, you will be responsible for triaging incoming referrals (from GPs, other hospitals, etc.) ensuring patients are directed to the most appropriate therapy service in a timely and efficient manner. The candidate is expected to have an understanding of community services and previous experience of working in the UK healthcare system. Triage work is based at the Ealing referral hub at St Bernard’s Hospital site (UB1 3EU) and clinics are held in one of our community sites in Southall. Staff will need to be flexible to support triage from Monday to Friday and work in clinics based on estate availability.
Main duties of the job
* Work independently at the referral hub to triage all referrals for community services received at the single point of access.
* Offer advice and guidance on the telephone for patients, referrers and carers as required.
* Prioritise and triage based on needs‑based assessment.
* Take succinct and clear documentation to support triage decisions and clinical work.
* Carry clinical case load in outpatient community rehab e.g. assessment and management of fatigue, brain fog and return‑to‑work support for long‑term conditions.
* Work alongside referral hub and other clinicians from community services to offer clinical triage and allocation to respective services.
* Support audits and training of other staff.
* MDT working.
* Write reports and letters.
* Undertake clinical assessment via telephone/video call as required to facilitate effective clinical decision‑making.
* Use DSE/computers for electronic triage on System One.

Job responsibilities
* Undertake clinical triage of therapy referrals made to the Referral Hub, deciding on the correct care pathway for each patient.
* Carry clinical caseload to support community rehab e.g. long‑COVID rehab.
* Work collaboratively within a multidisciplinary team.
* Work in partnership with Referral Hub staff and management to ensure that patients are allocated to the correct team in a timely way.
* Liaise with acute hospitals, local authorities and other stakeholders to obtain information and resolve queries.
* Work with COVID/ Therapy SPA Triage Lead and the management team to develop and enhance performance within therapy and long‑COVID triage and associated pathways.
* Participate in the ongoing monitoring and evaluation of the therapy triage, ensuring that all required data is collected and undertake audits as required.
* Provide an evidence‑based assessment, treatment and advisory service to clients with a variety of conditions.

Person Specification
Education and Qualifications
* Degree OT and current professional registration.
* Relevant postgraduate training.
Knowledge / Experience
* Trained or worked in the NHS.
* Experience of doing clinical triage.
* Broad undergraduate experience in core clinical areas: orthopaedics / long‑COVID / community therapy / neuro / care of the elderly.
* Long‑term condition rehab e.g. fatigue / brain fog management, return to work.
* Awareness of audit and evaluation.
* Understanding of clinical governance and its application to work area.
* Working knowledge of community services.
* Active contribution to training others.
Skills, Abilities & Attributes
* Excellent communication skills written and verbal.
* Good time management, organisational skills.
* Good problem‑solving skills, ability to prioritise based on needs‑based assessment.
* Good IT skills.
* Recognise potentially difficult situations and when to seek appropriate help.
* Experience in using Systmone.
