St Giles Trust seeks a Caseworker to support vulnerable 12-25-year-old victims of violence at Northwick Park Hospital. The role involves case management, advocacy, risk assessment, and inter-agency collaboration. Applicants should have relevant experience, resilience, and knowledge of youth issues. Responsible for:

Case management for victims of violence, aged 12-25 years

Hours:

Full Time, 35 hours per week Monday-Friday 9-5pm (occasionally may work passed 5PM due to needs of Clients)

Contract: Fixed Term 31 March 2026 (with the possibility of continuing beyond this date)

Grade:

Band 2

Location:

Based at Northwick Park Hospital (Watford RD Harrow HA1 3UJ) with support provided within surrounding areas.

Holiday:

30 days + bank and statutory holidays (pro-rated if part time)

Salary:

£27,000 per annum

Please note successful candidates should expect to be appointed at the starting point of the salary scale and consideration may be given to a higher salary depending on the experience of the individual.

About the Role: St Giles is expanding its delivery of hospital-based projects; this service is aimed at vulnerable 12 to 25-year-olds who attend the adult or paediatric A&E departments or Urgent Care Centres Northwick Park Hospital. We will work alongside hospital staff to proactively identify vulnerable young people and help them to access and engage the support they need to prevent any potential escalation of violence and reduce the risk of repeat victimisation or exploitation.

You will be embedded into the hospital, providing support for those young people admitted to emergency departments right through to their discharge back into the community. You will have experience of working with 'high risk,' vulnerable young people, be part of a multidisciplinary team, and have a flexible, empathetic approach to your work.

(1) Key Deliverables

* Key Deliverables
* To work as part of a multi-disciplinary team to identify and assess young victims of violence, with reference to St Giles' assessment practices
* To contribute to the services' out of hours rota as required
* To be provide support, advice and advocacy for children, young people and their families as they plan to be discharged from hospital back into the community
* To produce support plans and risk management plans based on assessments. To promote inter-agency collaboration in the assessment and planning process, and to include appropriate agencies in the delivery of the service
* To develop and maintain relationships with partner agencies, including community-based services such as police, YOT, Probation, children's services, local authorities and other voluntary sector organisations ensuring appropriate information is shared, supported referrals are made and avoiding duplication of support
* To deliver a holistic support service working solo or with colleagues as the situation dictates; providing a practical service that will include social support, housing support, accompanying to appointments, ETE options, benefits work, debt advice, assistance obtaining furniture, liaising with utilities, appearing in court, DIY work and cleaning
* To close cases efficiently and positively, identifying a referral route for children and young people that will identify agencies that can be used for ongoing support and agencies that can be used if serious problems develop in the future
* To promote the value of lifestyle changes to the children and young people being supported
* To actively contribute to the training and development of hospital staff to increase their awareness and
* them to better identify and support young victims of violence
* To accurately and regularly collect information and evidence to support the with the monitoring and the evaluation of the service
* To adhere to St Giles (and the relevant hospitals) policy's and procedures including safeguarding, risk management, lone working, GDPR and diversity & inclusion
* To invest in your personal development by attending core and additional training to keep abreast of issues facing children and young people such as Trauma Informed Training, Child Criminal (and sexual) exploitation and contextual safeguarding
* To carry out any other similar duties as required and be flexible to the changing needs of this new, fast paced service

What we are looking for from you – Person Specification, When completing your application form please address the points marked with (A) set out below.

An ideal Candidate will reside in the North West region of London, this due to the intensity of this role added with a long travel journey to NWPH and supporting CYP in the community we know this is not always sustainable. They will also have a good understanding of the diverse community in which our services provides support too, this also includes a good understanding of dominate youth culture. Will have substantial experience of supporting CYP, working in partnership with other agencies and can work to a high standard.

* Experience
* 1) Personal experience of the criminal justice system, lived experience of the issues facing this client group and/or experience of working with 'high risk', vulnerable children, young people and/or families (A)
* 2)Substantial experience of providing support, advice and advocacy, and communicating effectively, the needs of clients to other professionals (A)
* 3)Substantial experience of assessing the needs of children and young people who at risk of significant harm (A)
* 4)Substantial experience in working as part of a multiagency team, working together to achieve positive outcomes for young people and establishing links to further the aims of a project (A)
* 5) Significant experience of using support plans, to enable people to successfully access support services (A)
* 6)Substantial experience of engaging successfully with 'challenging' young people, for example people who have complex needs, people who are reluctant to discuss their needs, and people who are angry and confused (A)
* 7)Significant experience of assessing risk and implementing safety procedures when thinking about children and young people involved in or at risk of violence or exploitation (A)
* 8)Proven experience of working in a high intensity environment, with an ability to effectively manage your own wellbeing (A)
