Overview
The post holder will work within their clinical competencies to provide specialist pharmaceutical support to all practices of the PCN including working with members of the wider healthcare team to improve care and support safe and effective prescribing. The role involves moving between practices within the PCN and has a patient-facing element (face to face or telephone), including chronic disease management, repeat prescription management and structured medication reviews. It includes the management and review of patients with polypharmacy. The post holder will be proactively involved in quality improvement and audit and may manage aspects of the Quality and Outcomes Framework. The post holder will be supported to develop the role to improve care and outcomes for patients across the PCN.
Responsibilities
* Clinical medication reviews for patients within the PCN with single or multiple long term conditions where medicines optimisation is required
* To reconcile medicines following discharge from hospitals or intermediate care and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge and to reduce risk of readmission
* To be involved in multidisciplinary clinical meetings within the PCN where appropriate
* Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas
* Be a prescriber, or completing training to become prescribers, and work with and alongside the general practice team
* Be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially older people, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism (through STOMP Stop Over Medication Programme)
* Provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the PCN's practice(s) and to help in tackling inequalities
