Rhondda Cynon Taf CBC's Disabled Children with Complex Needs Service supports young people aged 16+ as they prepare for independence and transition from care. The 11+ Team provides tailored advice, pathway planning, and ongoing support to ensure young people are safe, included, and equipped for adulthood.
Personal Advisor Responsibilities
* Act as Personal Advisor for young people aged 16+ preparing to leave care.
* Support the development, review, and delivery of pathway plans.
* Assess needs relating to health, education, training, and finances.
* Provide direct support to young people living independently.
* Coordinate access to accommodation, education, and employment opportunities.
* Manage financial assessments and arrange payments in line with 16+ policies.
* Deliver individual and group sessions to build life skills and reduce isolation.
* Maintain regular contact and clear expectations around support.
* Work collaboratively with families, accommodation providers, and partner agencies.
* Ensure accurate, timely case recording and adherence to safeguarding procedures.
Qualifications
* Experience working directly with young people.
* Knowledge of adolescent development and safeguarding.
* Ability to assess needs and contribute to care and pathway planning.
* Strong communication, organisation, and record-keeping skills.
* Experience working with multi-agency partners.
* Understanding of equal opportunities and anti-oppressive practice.
* Ability to travel independently across RCT.
* Enhanced DBS required.
* Flexibility to work outside standard office hours when needed.
