Do you have a genuine passion for supporting people living with obesity? An exciting opportunity has arisen, for an enthusiastic and motivated band 4 Physical Activity Associate Practitioner, to join our dynamic team within Tier 3 Specialist Weight Management.
Responsibilities
* Work with the Tier 3 Specialist Weight Management Multi-Disciplinary Team (MDT) to support patients to lose weight and improve their health and wellbeing.
* Assess and provide tailored physical activity advice and support to patients who access the service.
* Identify, demonstrate, and observe specific, tailored exercises to meet the needs of the patient.
* Accurately record patient information within electronic patient records.
* Use a range of communication methods and behaviour change approaches.
* Collaborate with the Tier 3 Specialist Weight Management Physiotherapist for clinical support and supervision.
Qualifications
* Experience of supporting patients living with obesity and complex health conditions.
* Excellent time management skills.
* Good understanding of the complexity of obesity and the challenges associated with obesity and physical activity.
* Ability to use a range of communication methods and behaviour change approaches.
Location
You will be based in the vibrant, cultural city of Hull which is continuing to thrive and grow after being awarded City of Culture in 2017. The post will be based at Marfleet Health Centre in Hull, however the successful applicant will also be expected to work from various other bases across Hull and East Riding.
