Outreach Worker - Full Time, Permanent
Location: Portland House, Sheffield
Hours: 37 hours per week, Monday to Friday
About the role
Join the Home at Last team supporting adults with complex needs and rough sleeping. Collaborate with city agencies to provide long-term solutions. As an Outreach Recovery Worker, you will support homeless or vulnerably housed individuals to access substance misuse support, maintain accommodation, and transition into settled housing.
Your duties will include but are not limited to:
* Deliver satellite services at locations like GPs, probation offices, and hostels.
* Provide harm reduction, brief interventions, care planning, risk assessments, and case management.
* Utilize previous voluntary or paid experience in client-facing roles and active listening skills.
About you
* Experience managing a caseload.
* Knowledge or experience of substance misuse.
* Support experience with vulnerable individuals affected by alcohol and/or drugs.
* Comfortable working alone and in various settings.
* Relevant qualification in Health and Social Care.
* Proactive, empathetic, motivated, and passionate about high-quality care.
