Job Description: Social Worker/Care Manager Who You Are You are experienced with at least 2 years in a local authority, health, or voluntary sector setting, with a strong understanding of relevant legislation and a commitment to the principles of personalisation, independence, and choice. You have experience in working with older or physically disabled individuals, are knowledgeable in service provisions, and have expertise in multidisciplinary work and assessment tasks. Your background in social work allows you to communicate effectively and manage complex cases, all while thriving in a team-oriented, diverse environment. What the Job Involves This role involves assessing the needs of individuals with complex requirements, providing professional guidance, and producing person-centered support plans that uphold self-esteem and independence. You will conduct holistic assessments, perform risk assessments, and ensure the safeguarding of adults in accordance with the council's policy. Additionally, you'll manage a caseload, facilitate self-assessment, negotiate service delivery, and maintain accurate records. Collaboration in a multidisciplinary context forms part of your daily responsibilities, alongside supporting carers, making placement arrangements when necessary, and ensuring financial assessments and budgets are appropriately managed.
