We have an exciting role on offer within our Prevention and Partnership as an Reablement Officer. About Us Have you worked in care or cared for someone you know? Are you looking for an opportunity to learn new skills? If so, working as a community based Reablement Officer could be for you. It’s an exciting time to join our team as we continue to expand and identify opportunities to expand further. The key skills for a successful Reablement Officer are: Good communication (verbal and written) Able to adapt to rapidly changing circumstances Being a good listener Being honest, reliable and able to ask for help when needed Flexible rotas are available to include mornings, afternoons, evenings, weekends and Bank Holidays. The shift pattern will include shifts from 07:00 – 14:00 or 15:00 – 22:00. Some split shifts may also be available – you’ll receive your rota in advance. You must have a genuine interest in promoting independence and supporting customers to regain their confidence and skills. This may be after a period of rehabilitation in a residential setting or on returning home from hospital. We also receive a large number of community referrals for customers who may be struggling to manage at home with an identified need. We have a wide customer base which includes mental health, mild dementia, mild learning disabilities, old age and chaotic lifestyles. The broad criteria for Reablement is to be over 18yrs of age, a resident of Middlesbrough and have an identified need. A full clean driving licence, access to your own vehicle and business insurance is essential as the role requires travel throughout Middlesbrough. The main areas of responsibilities of the post include: Supporting customers with their medication (training will be given), washing and dressing, food preparation, outdoor and indoor mobility and daily household chores. To provide social and emotional support to customers. To observe and record customer progress with identified goals using our detailed support plan – training and support will be provided. To use technology and systems safely and effectively – training and support will be provided. To maintain records as required. To work collaboratively with your colleagues, Occupational Therapists, Case Manager and other professionals to achieve the best outcome for our customers. To follow policies and procedures of Middlesbrough Council including being responsible for your own, your colleagues and our customers’ Health and Safety, GDPR and Covid-19 policies and procedures. Having an understanding of CQC’s requirements. To undertake mandatory training including Food Hygiene, First Aid and Medication Management. Attend regular supervision sessions.
