In this role you will work as part of a dynamic multidisciplinary team including Occupational Therapists, Physiotherapists, Nurses, and Community Support Workers. Our goal is to help patients remain at home and avoid unnecessary hospital admissions. As a Therapy Assistant Practitioner you will manage a caseload of non‑complex patients under the guidance of qualified clinicians, delivering person‑centred care and rehabilitation.

Main Duties
* Provide person‑centred therapy‑related care to adults in their own homes and residential care.
* Drive a licence and travel around the community in own vehicle essential.
* Use a health coaching approach to support patients in achieving rehabilitation goals.
* Manage a caseload of non‑complex patients under supervision of OT/PT.
* Deliver therapy interventions: mobility and transfer practice; exercise and rehab programmes; daily living skills training; prescribe non‑complex equipment; complete holistic assessments.
