Do you hold a foundation degree or higher apprenticeship in a health‑related area?

Do you want to work with an innovative and supportive team to support patient independence?

In this role you will work as part of a dynamic multidisciplinary team including Occupational Therapists, Physiotherapists, Nurses, and Community Support Workers. Our goal is to help patients remain at home and avoid unnecessary hospital admissions. As a Therapy Assistant Practitioner you will manage a caseload of non‑complex patients under the guidance of qualified clinicians, delivering person‑centred care and rehabilitation.

Main Duties

* Provide person‑centred therapy‑related care to adults in their own homes and residential care.
* Drive a licence and travel around the community in own vehicle essential.
* Use a health coaching approach to support patients in achieving rehabilitation goals.
* Manage a caseload of non‑complex patients under supervision of OT/PT.
* Deliver therapy interventions: mobility and transfer practice; exercise and rehab programmes; daily living skills training; prescribe non‑complex equipment; complete holistic assessments.

Qualifications

* Foundation Degree in health related area (or equivalent higher apprenticeship)
* Evidence of personal development
* Care Certificate


Skills and Knowledge

* Good interpersonal skills
* Good ability to listen
* Good IT skills


Personal Attributes

* Ability to work with minimal direction
* Awareness of confidentiality requirements
* Approachable, reliable, and understands limits
* Willingness to travel across the community


Experience

* Experience in NHS/Social Care/Voluntary organisation
* Multidisciplinary teamwork experience
* Working knowledge of long‑term conditions and their effect on people
* Knowledge of therapy techniques and procedures


Disclosure and Barring Service Check

This post requires a DBS check under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
