Overview
Health Care Assistant - Neighbourhood Nursing Service. Full and part time hours available. Band 3 role within the Neighbourhood Nursing Service (NNS) and supporting Virtual Ward referrals in Barnsley area. NNS delivers nursing care to housebound patients, including wound management, chronic disease management, promotion of self-care and terminal/palliative care. The post involves working across planned nursing and crisis pathways, may require a seven-day week shift pattern including weekends and bank holidays. All employees are encouraged to be fully vaccinated against COVID-19. Responsibilities
* Contribute to the care planning process and provide high standards of safe and compassionate care.
* Work across planned and crisis pathways to support patients to remain well at home or in long-term accommodation and/or prevent hospital admission.
* Develop and maintain meaningful therapeutic relationships; ensure positive care experience and service user safety.
* Attend to service users personal care needs; undertake physical aspects of care where appropriate (e.g., moving & handling).
* Apply knowledge of long-term health conditions to provide advice and support.
* Exercise independence with supervision; use initiative and judgement when delivering tasks.
* Participate in training, clinical supervision and reflect on own practice.
* Participate in flexible rostering to provide 24-hour/7-day care as appropriate.
* Work in partnership with primary, secondary, social care and voluntary sectors to improve health and care, especially for the most vulnerable.
* You Will: Contribute to care planning and documentation; record clinical measurements accurately; identify risks and respond appropriately.
* Support service users to self-manage health and wellbeing; promote service user safety and dignity.
* Assist in managing challenging behaviour using de-escalation techniques; follow risk assessments and safety protocols.
* Move and handle service users safely; use equipment (e.g., hoists, wheelchairs) as appropriate and with proper instruction.
* Participate in a multidisciplinary team; adapt to flexible working patterns and different clinical areas as required.
* Support the Trust’s values and culture; maintain confidentiality and professional standards.
Qualifications and Requirements
* Essential – GCSEs A-D or 3-9 in Maths and English or equivalent; Apprenticeship level 3 or NVQ Level 3 in Health & Social Care or equivalent; or Level 2 with competencies at Level 3; Care Certificate or to be achieved within 12 weeks of commencement.
* Personal Attributes – Responsive, professional dress, valid driving licence and access to a car during the working day (reasonable adjustments considered).
* Physical Attributes – Satisfactory sickness record over previous 2 years (with fairness/equality considerations).
* Desirable – Level 4 in Health & Social Care; experience in the speciality area; willingness to undertake further study towards Level 4 qualification.
* Special Knowledge/Skills – Excellent customer care, flexible to 24-hour shifts, safe decision making, excellent communication, IT skills, confidentiality, resilience, non-judgemental approach, trust values, team working, and ability to identify risks.
* Training – Willing/able to undertake mandatory Trust training and ongoing professional development; knowledge of safety practices (e.g., Prevention & management of violence & aggression, moving and handling).
* Experience – Providing personal care and related procedures; working in a caring environment at Band 2 or equivalent; public-facing experience; effective teamwork; initiative and prioritisation; commitment to user/carer involvement.
