We are recruiting a Social Prescribing Link Worker for the East Herefordshire Primary Care Network (PCN). The role will provide personalised support to patients, helping them connect with services that meet their non-clinical needs. You will work as part of the multi‑disciplinary PCN Wellbeing Team, supporting frail patients and those at risk of health inequalities, and co‑designing plans with them to improve health and wellbeing.
Main duties of the job
Undertake holistic assessments of patients’ circumstances to determine their non‑clinical needs, co‑designing relevant actions with patients and ensuring maximum engagement in improving health and wellbeing.
Introduce or reconnect people to community groups and statutory services, and develop and maintain strong partnership relationships with a wide range of statutory and voluntary sector organisations.
Work with practices and the PCN team to proactively target local health inequalities and poorer health outcomes, while providing a coordinated experience of care and fostering patient choice, empowerment and independence.
Responsibilities
* Produce and signpost patients to community resources, maintaining up‑to‑date information.
* Act as an advocate and champion for the social prescribing service across the PCN, the public, and professionals through outreach and communication mechanisms.
* Co‑produce simple personalised care and support plans with patients, families and carers.
* Deliver group sessions aimed at reducing health inequalities and improving health outcomes.
* Maintain accurate and professional records, calendars and diaries for patient appointments.
* Ensure safeguarding, equal opportunity and confidentiality practices are followed.
* Participate in ongoing delivery, monitoring, and evaluation of the programme, providing regular reports to the PCN manager.
* Provide accurate feedback to referrers, including GPs and other statutory or voluntary agencies.
Qualifications
* Experience of working in the community to improve health and wellbeing.
