Location: Auxillium House, Legion way, Hereford
Hours: 37 hours (including some weekend cover)

Home First is Hoople Care’s Urgent Care and Reablement service, supporting adults to remain independent and return home safely following hospital admission or crisis. We work closely with health and social care partners to deliver timely, responsive, and person-centred support.
We are looking for an organised, proactive and adaptable Referral and Scheduling Coordinator to join our busy multidisciplinary team.
About the role
This is a varied and fast-paced role at the heart of the Home First service. You will be the first point of contact for all enquiries about referrals, coordinating assessments, and ensuring support visits are scheduled efficiently. You will play a key role in supporting managers to maximize service capacity, ensuring safe and timely discharges, and responding to urgent care referrals.
Key responsibilities
1. Act as the first point of contact for all enquiries about service referrals,
2. Support managers to manage referral waiting lists, ensuring timely responses and risk reduction
3. Plan, coordinate and update support visits, rotas and service schedules
4. Allocate work to Reablement Workers and Assessment and Review Officers
5. Maximise service capacity through effective scheduling, route planning and use of resources
6. Calculate reasonable travel times, mileage and staff rounds
7. Coordinate and allocate service assessments
8. Maintain accurate records across in-house databases and systems
9. Work closely with internal teams and external health and social care partners
10. Support with Quality Assurance gathering
11. Complete day to day administrative duties and coordination support to the service and staff
About you
You will be highly organised, calm under pressure and confident managing competing priorities in a changing environment. You will be able to think creatively to solve problems and work collaboratively with colleagues across health and social care.
Essential requirements
12. Level 3 qualification in Health and Social Care
13. Experience working with vulnerable adults in a health and/or social care setting
14. Excellent administrative and IT skills, including Microsoft Office and scheduling/rostering systems
15. Strong communication skills, both written and verbal
16. Ability to prioritise workloads and work to tight deadlines
17. Good geographical knowledge of Herefordshire
18. Flexible approach to working hours
Desirable
19. Experience in a care coordinator or domiciliary care role
20. Supervisory or team-leading experience
21. Experience working with referral management and scheduling systems
