Job summary
Hampshire and IOW (HIOW) Integrated Care Board (ICB) have recently set out our high-level ambitions by describing the strategic vision for primary and local care transformation over the next three years. Hampshire & Isle of Wight's population is ageing, with the 90+ age group expected to increase by 93% by 2043. As frailty and multimorbidity rise, there's a clear need for Integrated Neighbourhood Working (INW) to support independence and reduce utilisation of urgent and acute services within neighbourhoods and hospital settings.
Gosport Central PCN are looking for a qualified GP to lead the proactive care frailty team supporting with chronic disease and health inequalities projects to address challenges related to frailty, multimorbidity, and system pressures by keeping people healthier and independent for longer, reducing crisis-driven care, and ensuring equitable support for vulnerable populations.
You will be part of a team consisting of a Lead Nurse, Occupational Therapist, Physiotherapist, Paramedic and GP Assistant to help deliver the aims and objective of the scheme.
This is a great opportunity to join the Gosport Central Primary Care Network team that strive to deliver outstanding patient care to approximately 85,500 patients in Gosport. You will be an integral part of the team, working alongside our 5 member practices in the locality and would be part of a senior leadership team.
Main duties, tasks & skills required
* Provide high-quality patient care
* Provide leadership to our Pro-Active care Frailty team
* Conduct surgery, telephone, and video consultations, plus occasional home visits
* Liaise with colleagues and external providers to ensure seamless care
* Investigate and refer patients appropriately, in line with guidance
* Review results and correspondence promptly
* Collect and review data for audit and QOF purposes
* Maintain excellent communication with patients and carers
* Attend clinical, governance, and educational meetings
* Work collaboratively and effectively within the team
* Manage your own workload and resources effectively
* Attend required training events and feedback sessions
* Report and follow up on significant incidents

* GP with GMC registration
