This is a unique opportunity for a skilled, experienced, and dynamic health professional to lead the implementation of a new Primary Care Capabilities Framework for Inclusion Health, Prison, and Custody Suites.
You will work at a national level with a wide range of stakeholders under the direction of the Consultant in Public Health Medicine as part of a multi-skilled team. Your expertise will drive the framework's implementation across Wales. You should have substantial experience and knowledge of inclusion health in the community and/or prisons, with clinical credibility in these areas.
You will deliver this work within a challenging timeframe, requiring intellectual rigour, negotiation skills, flexibility, pragmatism, and effective time management.
You will collaborate with various stakeholders and senior leadership, including staff within Health Boards, prisons, primary care organizations, professional groups, and other key organizations.
Your responsibilities include formulating and coordinating complex implementation plans for the national rollout, managing interdependencies across programs, and re-prioritizing resources to ensure timely delivery aligned with local, organizational, and national strategies.
You will lead the development and implementation of national policies and service improvement initiatives related to inclusion health, translating national policies into actionable local strategies aligned with NHS Wales objectives and contributing to service transformation.
Further responsibilities involve strategic planning, stakeholder engagement, chairing senior meetings, and representing the work across various platforms, ensuring continuous improvement based on evidence and complex data interpretation.
While not having direct line management, you will provide leadership and guidance to project support staff, ensuring milestones are met and progress communicated effectively.
