Overview

An exciting opportunity has risen for a Band 3 Health Care Assistant on Jasmine Ward (Tolworth Hospital). Jasmine Ward is a 16 bedded unit that offers care and treatment for male and female patients over 75 years of age. The ward provides inpatient care for older people with mental health problems, assessment and treatment of older people with a range of diagnoses who require hospital care due to the severity of need. The service serves patients who cannot be cared for in the community or other settings due to the intensity and expertise of the care required. The ward is located in Red Lion Road, Surbiton Tolworth with good local links via underground and bus to outer London.

Responsibilities

1. Working under the guidance of a Registered Nurse/ AP/ NA/ B4, to implement the delivery of quality nursing care, contributing to physical, psychological, social, recreational and spiritual care in a recovery-focused way through therapeutic engagement and activities, and taking steps to address any issues that arise.
2. Leading on and undertaking activities to ensure the fundamentals of care are met – nutrition, hydration, hygiene, comfort, and emotional and social support.
3. Supporting junior staff and providing hands-on care to service users, including:
o Undertaking 1-1 engagement with service users in an honest, open and non-judgmental way.
o Recognising and valuing service users as individuals, maintaining the service user\'s respect and dignity, and ensuring care respects equality and diversity.
o Escalating concerns to a Registered Nurse.
o Monitoring and recording clinical observations (physical observations and vital signs) and escalating signs of deterioration or personal parameter deviations to the relevant professional (Band 4 or Registered Nurse).
o Implementing the principles of recovery: encouraging healthy lifestyles, facilitating psychosocial activities and other interests.
4. Escorting service users as required in line with Trust policies.
5. Facilitating admission and discharge of service users, ensuring needs are met including property checks, orientation to services, and ensuring community support needs are met under direction of a senior clinician.
6. Demonstrating an enhanced understanding of Mental Health signs and symptoms and how these may present in practice, including risk factors.
7. Recognising and responding to challenging behaviour in line with Trust policies and training, seeking supervision and guidance from a Senior Health Care Support Worker, Assistant Practitioner, or Registered Nurse.
8. Demonstrating knowledge of the fundamentals of the MHA, MCA, DOLS and other applicable legislation and how they are implemented in practice.
9. Contributing to effective communication with patients, carers, family, friends and colleagues. Engaging and listening carefully and actively, and using accessible communication methods. Developing communication skills to enable difficult conversations, acknowledging cultural differences and professional boundaries.
10. Demonstrating effective clinical communication and documentation: clear verbal and written handover of relevant information, identifying key information, describing symptoms of mental illness or deterioration, and escalating concerns promptly.

Contract and Working Hours

The post is for 37.5 hours per week and requires a range of shifts across 7 days.
