Clinical Nurse Specialist Diabetes

An exciting opportunity has arisen for a Band 7 Diabetes Specialist Nurse to join the Integrated Cornwall Diabetes Specialist Nursing Team, working primarily across Helston, Falmouth and surrounding areas, with ad hoc countywide cover.
The service is fully integrated with community services, primary care and secondary care, delivering a seamless patient journey. We are seeking an experienced and motivated Diabetes Specialist Nurse with a strong passion for improving outcomes for people living with diabetes in the community. Proven experience working within a Diabetes Team is essential.
The successful candidate will:
* Work autonomously as an advanced practitioner, delivering an expert, integrated diabetes service across community, primary and secondary care.
* Manage a defined caseload through nurse-led intermediate clinics within community hospitals and GP cluster sites, including inpatient community hospital visits.
* Provide comprehensive clinical and psychosocial support to patients and families.
* Lead service development and evidence-based practice in line with current research and national guidelines.
* Establish and maintain effective communication across the multidisciplinary team.
* Act as an educator for students, nursing staff, medics, dietetic and AHP colleagues.
* Support the DSN advice, guidance and referral service.

Main duties of the job
To provide a specialist nursing service and education to people with Diabetes, their carers, and healthcare teams across community, primary and secondary care settings within Cornwall. This will include managing a patient caseload, acting as an educational resource for service users / providers & promoting effective communication between all health professionals and the voluntary sector.
To assist with professional leadership and management of the Cornwall Adult Diabetes Nursing Service in conjunction with the Lead Clinical Nurse Specialist to provide the highest possible standards of care to the patients and their families.
Nurse led Intermediate clinics are run by the post holder at Community hospitals and GP cluster locations. The post holder also provides Community hospital in-patient visits where required, and support to Cornwall Foundation Trust HealthCare Professionals.

Person Specification
Education/Qualification and Relevant Experience
* Relevant first degree or evidence of working towards this.
* Substantial experience at a senior level within an adult care acute / community setting caring for people with Diabetes.
* Independent Non- Medical Prescribing Course, or willingness to undertake.
* Good written and verbal communication skills.
* Computer Literacy.
* Good organisation skills.
Knowledge and Abilities
* Knowledge of current issues in Diabetes care.
* Ability to work as team member & autonomously.
* Ability to cope under pressure.
