Join our Team: Together for Children Sunderland is on a transformative journey to make a real difference in the lives of children and young people in Sunderland. We are an outstanding Ofsted‑rated Children’s service. If you’re committed to helping children, families, and our community thrive and have the skills to support our mission, we would love to hear from you. Learn more about us here: Together for Children.

About the role

You will work as part of a team based within the Paediatric Emergency Department (PED) at Sunderland Royal Hospital to reduce unnecessary re‑attendance at the PED. Your focus will be on improving awareness of and access to broader health and wellbeing support available in the local neighbourhood, including pharmacy, GP, and Family Hub services. You will work with and support families to ensure they fully understand information from PED staff on how to manage their child’s condition and signpost families to additional services as required. This role acts as a conduit, bringing together advice and support offered by system partners into one place, helping parent/carers understand what services are available and how to access them.

The successful candidate will contribute to the improvement of outcomes for all children and their families in Sunderland, providing the right support to help children and families thrive. The role will also support the relevant Company (TfC) Key Performance Indicators and Family Hub outcomes.

This will bring a new multi‑agency approach to Sunderland PED. This post is exempt from the Rehabilitation of Offenders Act 1974 and therefore will be subject to an Enhanced DBS check from the Disclosure and Barring Service. Due to recent changes in UKVI rules, this role does not meet the criteria for Skilled Worker visa sponsorship under the updated eligibility requirements effective 22 July 2025.

We want you to apply if you…

* Hold NVQ Level 3 in an appropriate discipline, or possess current significant experience in a similar role.
* Have experience working and engaging with children, young people and their families in a challenging and pressured environment within a multi‑agency approach.
* Have good communication and record‑keeping skills.
