Job Overview

Learning Disability Crisis Response Intensive Support team (LD CRIST)

The LD CRIST are a multi disciplinary team, with learning disability & mental health nurses, psychologist, speech and language therapists, occupational therapist and psychiatrist.

The LD CRIST provide a service 7 days a week between the hours of 08:00-21:00hrs including bank holidays.

Opportunities have arisen for Band 3 Community Learning Disability Support Worker to join LPT to support people and their families in a community setting. The primary aim of the team is to prevent unnecessary hospital admissions. LD CRIST will provide rapid assessment, intensive support to people with a learning disability and behaviours that challenge or mental health conditions.

The principle role of the Senior Healthcare Assistant is to assist the healthcare team in the provision and delivery of programmes of patient care under the indirect supervision of registered practitioners. This includes: Taking physical or psychological measurements from named clients/service users as delegated by the team.

Job Description Summary (Main duties and responsibilities)

* You will contribute to assessments and interventions including undertaking direct observations, completing specialist risk assessments and care planning.
* Supporting clients/service users in maintaining their health and wellbeing.
* Undertaking specified and delegated clinical and therapeutic activities.
* Under the supervision of healthcare professionals assist the healthcare team with the planning, organising and provision of clinical care.
* Ensuring high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals.
* Assisting Registered Professionals in the implementation and evaluation of individualised care plans.
* Accurately recording patient information and provides support and supervision to other colleagues and learners as appropriate.

The ideal candidate will have had previous experience of working with adults or children with Learning Disabilities in a community setting.

You must have excellent communication/interpersonal skills and the ability to work on your own initiative as well as part of a team.

Please note that this role requires a full UK driving licence and access to a vehicle to use for work during working hours.
