Who You Are We are seeking an experienced agency social worker to join our West Learning Disability Team, working primarily in the Rhondda area within the Rhondda Cynon Taf borough. You are someone who is dedicated to promoting the wellbeing of adults with learning disabilities and collaborating effectively within an experienced and supportive team. What the Job Involves Your role involves safeguarding and promoting the wellbeing of adults through high-quality social work practices, including assessment, care planning, and review. You will conduct assessments in line with the Social Services and Well-Being Act (Wales) 2014, and perform Mental Capacity Assessments within the framework of the Mental Capacity Act 2005. Key responsibilities include: Developing and reviewing care and support plans. Collaborating with other agencies using a multi-disciplinary approach. Recording and updating records according to departmental policy. Traveling independently throughout the service area. Working remotely with one mandatory office day per week. Skills Strong social work practice Proficiency in assessment and care planning Experience with Mental Capacity Act 2005 Risk assessment and management Collaboration and multi-disciplinary teamwork Effective record-keeping skills Ability to travel independently
