Responsible for providing a high quality and comprehensive social work service to adults, carers and the community. Ability to deal with more complex/unusual cases assessing and managing risks and to give evidence in court in relation to care or other proceedings.
Accountabilities:
Undertake the assessment of service users and carer needs using a person centred approach in accordance with agreed statutory and service procedures
Use management information systems to find, analyse and update information, ensuring compliance with service procedures and data protection legislation
Devise and implement person-centred support plans, negotiating a range of appropriate services and interventions to meet assessed needs and risks
Monitor and contribute to the review of plans relating to the assessed needs of service users, their families and carers in accordance with directorate procedures
Use a Safeguarding framework to investigate and manage risks where individuals have experienced or are likely to experience significant harm
Provide concise written/verbal reports which can be used within the legal context
Identify and implement strategies for responding appropriately to concerns about practice or procedures, seeking guidance if required.
Convene, chair and participate in meetings / forums in line with procedures and develop effective working relationships to foster holistic and integrated practices
Help improve outcomes for people from diverse and disadvantaged communities
Work and develop own practice / capabilities within national professional standards
Requirements
Qualified Social Worker (Degree/Diploma in Social Work).
Registration with Social Work England.
