Responsibilities

* Follow care plans meticulously to ensure personalised support for each patient.

* Collaborate effectively with other healthcare professionals to deliver comprehensive care.
* Drive patients to appointments or activities as needed, ensuring their comfort and safety during transport.
Requirements
* Previous experience in home care setting is highly desirable.
* A valid driving licence is preferred for transporting patients.
* Proficient in English, both spoken and written, to ensure clear communication with patients and team members.
* Basic IT skills for maintaining patient records and documentation.
* Excellent communication skills, demonstrating empathy and understanding towards patients' needs.
* Ability to work effectively as part of a team while also being self-motivated in individual tasks.
