Job Overview

We are seeking an experienced individual to support the next stages in the development of the newly established CAMHS transitions service. This role will be a pilot, running for 12 months initially, with potential for becoming permanent. The role supports young people from CAMHS inpatient settings through recovery, employing a holistic, social prescribing model to enhance community, education, employment, and wellbeing.


Responsibilities

* Promote and facilitate the participation of service users and carers in their own care and in the planning and development of services.
* Take clinical lead and full line management responsibility for the transition service, ensuring high-quality, young‑person focused care.
* Lead and manage referrals into the Transition Service.
* Work across inpatient and community settings, coordinating daily communications with community CAMHS, Children’s Services, education providers, third‑party services, and adult services.


Qualifications

* Core professional registration in a relevant health profession.
* Post‑graduate development and training in Child and Adolescent Mental Health Services (CYPMHS).
* Experience working with CYPMHS and therapeutic work with complex cases.
* Knowledge of NICE guidelines and therapeutic interventions.
* Evidence of group work experience or ability to run groups.
