Band 6 Occupational Therapist - Discharge to Assess Secondment / Fixed Term Contract considered! Countywide - We have Occupational Therapist opportunities across Somerset.
We are expanding and seeking dynamic and inspiring Occupational Therapists to join the Discharge to Assess (D2A) service, part of the wider Intermediate Care service for Somerset. You will gain in-depth experience in community working and supporting individuals within their own homes. Successful candidates will receive support from a Senior Occupational Therapist and an Advanced Practitioner/Clinical Specialist within the team.
Our service has helped many regain independence at home following hospital stays. It enables patients to return home quicker, reducing the need for long hospital waits for care packages. Due to positive outcomes and ongoing investment, we are expanding our team with several new posts available.
Main duties of the role
You will perform specific therapy assessments, including goal setting, treatment planning, risk assessment, and promoting independent functioning. A person-centred approach, prioritisation, and flexibility are essential, along with collaboration with provider teams and rehabilitation practitioners.

Person Specification
Qualifications (Essential)
* Diploma/Degree in Occupational Therapy
* HCPC registration
* Knowledge of community Occupational Therapy role
* Driving license

* Excellent verbal and written communication skills for complex patient needs, potentially distressing situations
* Advanced clinical reasoning for treatment planning and interdisciplinary collaboration
Experience (Essential)
