Responsibilities:
Deliver personalized support and assistance to clients with daily living activities, including personal care, meal preparation, and household tasks.
Build trusting relationships with clients, offering companionship, emotional support, and encouragement.
Assist clients in accessing community resources and participating in social and recreational activities.
Collaborate with multidisciplinary teams to develop and implement individualized care plans tailored to each client’s needs and goals.
Maintain accurate documentation of client interactions, progress, and any incidents or concerns.
Qualifications:
Compassionate and empathetic attitude towards supporting individuals in need.
Strong communication and interpersonal skills, with the ability to engage effectively with clients and team members.
Previous experience in a similar role or working with vulnerable populations preferred.
Ability to work independently and as part of a team.
Flexibility to work various shifts, including evenings, weekends, and holidays, as needed.
