Responsibilities
* Provide structured, strength-based, reablement-focused support to individuals with mental health issues.
* Collaborate closely with multi-agency partners to ensure a holistic approach to community support and prevention.
* Conduct thorough risk assessments and enablement strategies to promote recovery and independence.
* Uphold mental health legislation and promote human rights of individuals.
* Develop and enhance pathways that provide alternatives to hospital admissions, focusing on community-based solutions.
Qualifications
* Proven experience working with individuals with mental health issues.
* Solid understanding of reablement principles and risk assessment.
* Relevant qualifications in social care or a related field are desirable.
