The role is to work closely with our Practice Pharmacy Team and the overall clinical team within the Practice to identify and manage our registered patients, ensuring that appropriate support is provided and that healthcare and disease management are effectively coordinated. This could involve support for chronic disease management, screening programs, or software systems used to support patients.
Main duties of the job
Care coordinators will:
1. Proactively identify and work with a cohort of people to support their care requirements, using available decision support aids.
2. Bring together all of a person's identified care and support needs and explore their options to meet these in a single, personalised care and support plan, in line with best practice.
3. Help people manage their needs, answer queries, and support them in making appointments.
4. Raise awareness of shared decision-making and decision support tools, assisting people to be better prepared for shared decision-making conversations.
5. Ensure people have access to high-quality information to help them make informed choices about their care.
6. Assist people in accessing self-management, peer support, or interventions that support their health and wellbeing.
7. Explore and assist people in accessing personal health budgets where appropriate.
8. Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care roles.
