Larksfield & Arlesey Medical Partnership is seeking a motivated and experienced Advanced Nurse Practitioner (ANP) or Emergency Care Practitioner (ECP) with independent prescribing rights to join our friendly and forward‑thinking primary care team. This is an excellent opportunity to work autonomously while being fully supported within a well‑established GP partnership.
Main duties of the job
The post holder will deliver high‑quality, patient‑centred care through advanced clinical assessment, diagnosis, and management of acute and chronic conditions.
* Independent assessment, diagnosis and treatment of patients
* Prescribing and medication reviews in line with current guidelines
* Managing same‑day urgent and routine appointments
* Working collaboratively with GPs, nurses and the wider multidisciplinary team
* Contributing to service development, quality improvement and clinical governance

Job responsibilities
The post holder will work as an autonomous clinician within the GP practice, providing advanced clinical care to patients presenting with acute, chronic and complex health needs. The role requires independent prescribing, advanced clinical assessment skills and the ability to manage patients across the lifespan within a primary care setting.
The ANP/ECP will work collaboratively with GPs, nurses and the wider multidisciplinary team to ensure safe, effective and patient‑centred care, contributing to service development and quality improvement within the practice.
Clinical responsibilities
* Undertake comprehensive clinical assessments, including history taking, physical examination and diagnosis
* Manage undifferentiated and differentiated presentations independently
* Assess, diagnose, treat and discharge or refer patients as appropriate
* Prescribe medications independently in line with clinical guidelines and best practice
* Manage acute, urgent and long‑term conditions
* Order, interpret and act upon diagnostic tests and investigations
* Provide health promotion, disease prevention and patient education
Care management
* Provide same‑day urgent care appointments and routine clinical appointments as required
* Refer patients to secondary care, community services or allied health professionals where appropriate
* Ensure continuity of care through appropriate follow‑up and safety‑netting
* Maintain accurate, clear and contemporaneous clinical records
Multidisciplinary working
* Work collaboratively with GPs, practice nurses, HCAs, pharmacists and the wider PCN team
* Participate in MDT meetings and clinical discussions
* Support and mentor junior staff where appropriate
Quality, governance and safety
* Work within practice policies, procedures and clinical governance frameworks
* Adhere to safeguarding policies for children and vulnerable adults
* Participate in clinical audit, quality improvement and service development initiatives
* Maintain professional registration, revalidation and mandatory training
Professional development
* Maintain and develop clinical competence through CPD
* Keep up to date with evidence‑based practice and national guidance
* Actively contribute to a learning and supportive practice environment
Information governance
* Ensure patient confidentiality is maintained at all times
* Comply with GDPR, data protection and Caldicott principles
* Use clinical systems appropriately and securely
Health & safety
* Work in accordance with Health & Safety legislation and practice policies
* Report incidents, near misses and risks appropriately
Flexibility

Person specification
Experience
* Significant post‑registration experience in urgent care, primary care, emergency care or general practice
* Experience of autonomous clinical practice
* Experience in assessment, diagnosis and management of acute presentations
* Experience of prescribing and medication management
* Experience working in general practice or primary care
* Experience managing long‑term conditions
* Experience working within a multidisciplinary team in an NHS setting
Qualifications
* Registered Nurse (NMC) or Registered Paramedic (HCPC)
* MSc level qualification in Advanced Clinical Practice or working towards one
* Independent Non‑Medical Prescribing qualification V300
* Evidence of ongoing Continuing Professional Development (CPD)
* Minor illness trained
* Advanced Clinical Practice qualification fully completed
* Teaching, mentoring or clinical supervision training
