You’ll also support our front-of-house duty function, helping to triage and screen referrals, respond to initial enquiries, and ensure carers are directed to the most appropriate support.

Key Responsibilities

* provide one-to-one support and guidance to unpaid carers
* assist in developing and implementing personalised carer support plans
* support front-of-house duty to triage and screen incoming referrals
* coordinate and participate in outreach and engagement activities
* maintain accurate records and contribute to service monitoring
* collaborate with health, social care, and voluntary sector partners

Our ideal candidate will have

* knowledge of the Care Act 2014 and assessment processes
