Job Summary
Working as a senior member of an established Community Mental Health Team in the East Locality of Doncaster providing a high standard of safe, robust and evidence based clinical practice.
Working with complex organic and functional diagnosis within the community setting, liaising and integrating with both statutory and voluntary organisations in support of the specific client group.
Main Duties
Promote and maintain the highest attainable standard of nursing care in a community setting.
Support the band 6 nursing team with the implementation of evidence based interventions from a plan of care.
Perform re‑assessments of clients to include risk and crisis plans.
Maintain close working relationships with referring agencies.
Deputise for the Team Leader when required.
Carry a caseload of patients with both functional and organic issues as identified through the team systems process.

Pay and Contract
Band 5, Salary £31,049 to £37,796 a year per annum, Fixed term, 11 months, Full-time, Flexible working.

Qualifications (Essential)
* RMN, Nursing Qualification.
Qualifications (Desirable)
* Registered teaching /training qualification.
Experience (Essential)
* Demonstrable experience working successfully with older people suffering with dementia and functional mental illness.
Knowledge (Essential)
* Experience of cognitive testing, complex assessments, working with family and carers.
* Management of cognitive enhancer treatments.
Skills (Essential)
* Advanced comprehensive communication skills.
* Clear/concise command of English written and oral.
* Ability to formulate plans for service strategy.
* Coordination of multi‑professional team activities.
* Proficient with IT skills.
* Displays a positive attitude, motivated.
Skills (Desirable)
* Specialised skills e.g. appraising, interviewing, counselling skills.
* Report writing, knowledge of policies and procedures for the Trust.
