We are seeking a compassionate, reliable and motivated Community Support Worker to work in the Adult Services Contact Team.
The successful candidate will be educated to NVQ Level 3 in Social Care or hold a Diploma in Health and Social Care.
You will play a key role in helping individuals to remain in their local community with support by providing advice and signposting.
The role involves you answering telephone calls and giving advice as well as signposting people to community resources. You will be expected to complete Care Act assessments and support people in a strength based way.
Within the role, you will enable people to access opportunities that will improve their wellbeing and quality of life.
