RM214203 - Senior Community Nurse - Band 6
Aberdeen City Health and Social Care Partnership is seeking a highly motivated Senior Community Nurse for the Community Nursing Outreach Team (CNOT).
An aim of CNOT is to address health inequalities within Aberdeen City. The team work collaboratively with Marywell practice and Integrated Drug Services (IDS) providing physical health assessment and nursing care for people who may have a history of alcohol and substance use or are less likely to engage with traditional healthcare services.
The service operates Monday Friday 0830-1700.
Clinics are currently provided at the Timmermarket building within Marywell Practice and in various outreach locations across the city.
Post holders are required to work autonomously, make clinical decisions and escalate concern where necessary. A background in primary care is advantageous with experience in chronic disease management and sound knowledge of GP practice IT systems.
If you are an experienced Registered Nurse and you would welcome the opportunity to be part of a developing service, ensuring best possible multi-disciplinary joint working and care for vulnerable people, this may be the role for you.
The successful candidate must have access to a car for work purposes and hold a full UK driving licence as travel throughout Aberdeen city area is essential.
